Welcome to the Chelan-Douglas Health District Birth and Death Certificate online ordering system. Requests will be processed from 9:00 AM to 5:00 PM on Monday through Thursday and from 9:00 AM to 12:00 PM on Fridays.
GETTING A BIRTH OR DEATH CERTIFICATE
Birth Certificates
- The office only issues certificates for births that occurred in Washington from 1907 to the present.
- For births outside the State of Washington, please contact the state in which the event occurred.
- Only qualified applicants are eligible to receive a copy of a certificate.
New Birth Certificates
- Certificates for new births typically takes up to 2 weeks to become available. While Chelan-Douglas Health District strives to provide this service as quickly as possible, the information must first be received from the birthing hospital (or center) and uploaded into the State of Washington records system.
- The Office of Vital Records processes orders within 2 business days once the information is available.
Death Certificates
- This office only issues certificates for deaths that occurred recently.
- For deaths that occurred before 1/1/2015, contact the State of Washington Department of Health.
- The State of Washington Department of Health's Center for Health Statistics issues certificates for all deaths occurring in Washington from 1907 to the present.
- For deaths outside the State of Washington, please contact the state in which the event occurred.
- Only qualified applicants are eligible to receive a copy of a certificate.
Attention Applicants
- If you are applying for your own Birth Certificate and you have changed your name since birth, (Legal Name Change, Adoption) please enter the reason for your name change in the 'Special Instructions' field on Step 4 of the application.

Same-Day Expedited - Same-day orders are available for pick up without waiting in line. Same day orders submitted after 3:30 PM will be ready the next business day. Expedited same-day service is NOT available on Fridays Records can be picked up at 200 Valley Mall Parkway, East Wenatchee, WA 98802. |
